Social Support for Seniors

Is there a cost to attend?

CHSP Group participants pay for their own meals and are asked to pay a contribution of $8 or $12 for minibus outings which assists with the cost of reimbursing our amazing volunteers for their out-of-pocket costs.

If you have a Home Care Package or Support at Home funding, we can contact your service provider to discuss your participation in our groups.

What types of activities are offered?

We offer a number of small group activities to cater to a range of interests and backgrounds. Call us to discuss the group options for you/your family member on 9856 1500. Groups include:

  • Light Walking Group
  • Cafe Perks
  • Carramar Friendship Group
  • Happy Wanderers
  • Chinese Walking Group
  • Saturday Lunch Group
  • Movie Group
  • Special Outings

What does Doncare's Social Support Program do?

Doncare provides volunteer supported social and recreational outings for seniors living in their own homes and who are or could be at risk of becoming isolated socially. Our aim is to help older people to enjoy a satisfying and rewarding quality of life by providing a responsive, flexible service that will assist individuals to retain their independence for as long as possible. Who is eligible to join our Social Support Groups?

Doncare’s SSP is subsidised by the Commonwealth Home Support Programme (CHSP), funded by the Department of Health, Disability and Ageing.

The program is available to people aged 65 years and over (or 50 years and over for Aboriginal and Torres Strait Islander people) who have been assessed as eligible for CHSP services through My Aged Care.

The Victorian Department of Health provides limited funding for people under the age of 65 who live with disabilities but are not eligible for the National Disability Insurance Scheme (NDIS), to participate in Doncare’s Social Support Groups. Contact the SSP Team for more information here.

Do i need a My Aged Care referral?

People receiving support from a Home Care Package or Support at Home program may be able to join our groups. Please contact our Social Support Program Team Leader to discuss.

Referrals are received through My Aged Care. Visit www.myagedcare.gov.au or call 1800 200 422 for more information or to request an assessment and referral. Contact our friendly SSP team if you require assistance to guide you through this process here.

I no longer drive, can transport be arranged?
Yes, our wonderful volunteers provide door to door transport for participants. Our volunteers will assist with stowing any walking aids in the vehicle.
